Glee has released a memorial card for the late Cory Monteith. It shows the actor in a number of scenes from the television series’ past episodes. The video ends with the words “Cory Monteith Forever in our hearts.”

Cory was found dead in his hotel room in Vancouver the afternoon of July 13. His autopsy revealed that he died of “mixed drug toxicity, involving heroin and alcohol.”
